New! 2022-2023 Contracts: All contracts require quarterly financial, narrative, and materials use reports to be submitted according to the table below.

Quarterly Financial Reports

Quarterly financial reports should be sent to the Agreement Administrator listed in Rider B #6 of the Agreement to Purchase Services (your contract) with a copy sent to Karyn Butts (karyn.butts@maine.gov).  Get the DHHS contact listing for staff which provides the CRM and SPOC assigned to each vendor that currently has a contract with DHHS.

Quarterly Narrative Reports and Materials Use Reports

Narrative reports should include a description of progess made toward meeting contract deliverables, and must be accompanied by a materials use form to track your distribution of small media materials. Please use the reporting forms below for your reports.

Local Capacity Self-Assessment Tool

New! All lead poisoning prevention partners are required to complete and submit a local capacity self-assessment annually. Please use one of the tools below to complete your assessments. Choose the One City version of the tool if you only provide services in one city or if you would like to do a combined assessment of the two cities you serve. Choose the Two Cities version of the tool if you would like to assess capacity in two cities separately. Completed self-assessments are due by April 1 of each year. Send completed self-assessemnts to karyn.butts@maine.gov.
