Maine MAPP Toolkit
MAPP stands for “Mobilizing for Action through Planning and Partnerships.
It is a public health planning framework for the protection and improvement of community health used all over the country and here in Maine. It was developed by NACCHO (National Association of City and County Health Officers)
The Maine MAPP Toolkit (PDF* 32MB) was developed by the Division of Local Public Health/Maine CDC with funding from several federal funding sources. It is adapted from elements of NACCHO’s “Achieving Healthier Communities through MAPP: A User’s Handbook”, originally funded by the Public Health Practice Office of the US Center for Disease Control and Prevention (CDC).
MAPP uses a self-assessment approach and reflects the practice needs of Maine’s Healthy Maine Partnerships who are implementing the MAPP process to develop Community Health Improvement Plans.
