EF-M-13 Excel Spreadsheet Instructions:

This version of the EF-M-13Excel Spreadsheet is designed to allow the M.I.S. Team to upload the pupil data that has been entered into the appropriate pages for your school.

The EF-M-11Excel Spreadsheet is available from the Department of Education homepage: http://www.maine.gov/education/forms/privateonly.htm or by contacting the M.I.S. Team at (207) 624-6790.

Submitting this spreadsheet by e-mail:

This spreadsheet is in MS Office Excel version 97-2003.
Please attach the spreadsheet to an e-mail and send to: GPA.DOE@maine.gov

Please comply with the following instructions:

Please complete all yellow shaded areas on the tab called "Signature Page"
Complete Section 1, Section 2, and Section 3 on the tab called "Sections 1-2-3"
Do not type anything in the CODE columns on the left side of each section.
If more cells are rows are needed, use the Supplemental tabs being sure to select the correct section for supplemental data.
All totals are automatically calculated. These cells have been protected to prevent accidental entering of data in these cells.

Supplemental 1a and 1b pages: These sheets are additional space for Section 1.

Supplemental 2a and 2b pages: These sheets are additional space for Section 2.

Signature page: A signed printed copy of the Signature page MUST be mailed or faxed to the M.I.S. Team. This page must indicate whether this form is being submitted on the accompanying diskette or being e-mailed.
