What is FST? When can FST be used? ~Multiple systems are involved, without benefit of a comprehensive plan ~Two or more agencies are providing services ~The family is experiencing significant difficulty in several of the nine life domains ~There has been long term involvement with multiple systems without significant improvement in quality of life for the family
How does FST work? ~An FST Facilitator meets with the family to develop a strategy that brings all relevant people to the table ~Using a process that identifies the family's strengths and needs, the Family and Systems Teams develops a comprehensive plan involving the family, providers, and natural supports ~A Plan Coordinator designated by the Team continues to work with the group to coordinate service delivery and fulfill the plan's objective
