$350 renewal fee
$75 late fee

TERM

Active and non-active licenses must be renewed annually on or before April 1st of each year.

REQUIREMENTS

License renewal applications will be mailed approximately sixty (60) days prior to April 1st.

A late fee of $75 is assessed for renewal applications received after the expiration date.

A person who submits an application for renewal more than sixty (60) days after the license expiration date is subject to all requirements governing new applicants.

PLEASE ALLOW 7 TO 10 BUSINESS DAYS FOR PROCESSING LICENSE RENEWALS.

CONTINUING EDUCATION

Documentation of 25 hours of continuing education is due at the time of renewal. At least fifteen (15) hours of continuing education must be in diagnosis and treatment of ocular diseases. A maximum of two (2) hours in practice management and five (5) hours on-line courses.

10 DAY NOTIFICATION REPQUIREMENT (Effective 9/20/07)

Pursuant to Title 10, §8003-G(2):
All licensees and applicants for licensure must report in writing any of the following to the office, board or commission no later than 10 days after the change or event, as the case may be:
a. Change of name or address;
b. Criminal conviction;
c. Revocation, suspension, or other disciplinary action taken in this or any other jurisdiction against any occupational or professional license held by the applicant or licensee; or
d. Any material change in the conditions or qualifications set forth in the original application for licensure submitted to the office, board or commission.

The Board may waive the continuing education requirement in cases of illness or undue hardship.
