National UCC Forms

 

*** New Uniform Commercial Code (UCC) Forms effective July 1, 2013 ***

Due to recent law changes, effective July 1, 2013 new UCC forms have been adopted.  The new financing statement forms will be available on this website no later than June 28, 2013.  The new forms cannot be accepted for filing until July 1, 2013.  A thirty (30) day grace period will be provided beginning July 1, 2013 and ending July 31, 2013, during which the previous versions of the forms will be accepted by the filing office.  Beginning on August 1, 2013, only the new forms will be accepted and the filing office will refuse to accept all other forms.

 

The Maine Secretary of State will only accept for filing the UCC forms that were most recently adopted by the International Association of Commercial Administrators. These forms are provided below in Acrobat (.PDF) format. If you wish to complete the forms on-screen, you need to download a version of the Adobe Reader by visiting the Adobe Website.  To use one of these fillable forms, simply complete on-screen, print and mail to our office together with the proper filing fee.

NOTE: UCC filings are public records. Please do not put people at risk of identity theft by including social security numbers on any documents for filing with the Secretary of State.

The UCC-1 and UCC-3 Additional Party Form may only be used in conjunction with either the UCC-1 or UCC-3 form to add multiple debtors or secured parties.

Please do not submit copies of UCC forms or return envelopes. The filing office will generate and mail a filing acknowledgment (if item A & B is completed) using envelopes supplied by the filing office.
