Candidate Petition Forms
Candidate petitions are available to download and print at the links below, or in printed form through the Division of Elections. Printed petitions may be picked up by candidates or by another person on behalf of the candidate, or they can be mailed. Candidates requesting pre-filled forms should contact the Elections Division.
Petitions must be printed double-sided on 8.5 x 14 inch (legal size) paper so that:
- The Secretary of State can confirm that all signers were able to see the candidate information on the front side before signing
- The text is large enough for voters to read easily
- The petition lines are large enough for voters to keep their information on one line
Primary Candidate Consent Forms
The candidate consent form is a separate document from the candidate petition. The candidate must submit the consent form with the petitions when they are submitted to the Secretary of State. Each consent form is specific to the office being sought.
